*Please note: if you are using a debit/credit card a $5 hold will be placed on your card at the time of the transaction. This is not the amount lost. The amount lost is the price of the product itself. Please allow 48 hours for the product’s amount to show on your account.
Once the form is completed, you will receive an automated response via email that you form has been received. Please check with one of these locations for pick up.
